Abstract
The immediate objective of most blood transfusion is to increase the oxygen-carrying capacity of the patient's circulation. The common practice of transfusion of whole blood has been criticized as wasteful. The use of packed red cells allows the
preparation of whole blood component, primarily platelets and fresh-frozen plasma, for those patients with specialized bleeding disorders and who do not necessarily need an increase in oxygen-carrying capacity.
The authors observed the changes in component transfusion from 1988 through 1992 at Dongsan Hospital. Total units of blood transfused for 5 years were 125,788 units and the rate of blood component transfusion was 58.2% in 1933, and it increased
from
1933 to 1992 (97.5%).
Large annual increases in the use of packed red cells, platelet concentrates and fresh-frozen plasma occured during 5 years. The rate of blood component transfusion was the highest in department of internal medicine. And the lowest in department
of
neurosurgery.
The rate of blood returned to the blood bank was the lowest in department of internal medicine and the highest in department of urology. Crossmatch to transfusion ratio was 1.3:1 in 1988 and gradually decreased from 1933 to 1992 (1.1:1).
